Seven HCA Virginia hospitals awarded 'A' rating in Leapfrog Group's fall 2025 Hospital Safety Grade

Fall 2025 Safety Grade is a national recognition for commitment to patient safety.

Seven HCA Virginia hospitals earned an “A” Hospital Safety Grade from The Leapfrog Group, a national nonprofit watchdog that sets standards for excellence in patient care.

The HCA Virginia hospitals that earned an “A” grade for meeting some of the industry’s highest standards are:

  • Henrico Doctors' Hospital
  • Parham Doctors' Hospital
  • Retreat Doctors' Hospital
  • LewisGale Hospital - Alleghany
  • LewisGale Hospital - Pulaski
  • Reston Hospital Center
  • StoneSprings Hospital Center

The Leapfrog Group is an independent national nonprofit watchdog focused on patient safety. This national distinction recognizes the hospitals’ achievements protecting patients from harm and providing safe healthcare.

Leapfrog assigns an ‘A’, ‘B’, ‘C’, ‘D’ or ‘F’ grade to general hospitals across the country based on their performance in preventing medical errors, injuries, accidents, infections and other harm to patients in their care.

 The latest recognitions earned three HCA Virginia hospitals a "Straight A" designation for five or more consecutive cycles of A ratings:

  • Henrico Doctors' Hospital
  • Parham Doctors' Hospital
  • Reston Hospital Center

“Patient safety is always a top priority for our hospital care teams and we are honored to receive these recognitions for going above and beyond,” said Jessica Kirby, HCA Healthcare Capital Division vice president of quality. “We are proud of our dedicated physicians, nurses, and colleagues who provide our patients with exceptional care each day.”

“Earning an ‘A’ Grade means these HCA Virginia hospitals made a true commitment to put patient safety first,” said Leah Binder, president and CEO of The Leapfrog Group. “We congratulate the leadership, Board, clinicians, staff and volunteers that all had a role to play in this achievement.”

The Leapfrog Hospital Safety Grade stands as the only hospital ratings program focused solely on preventable medical errors, infections and injuries that kill more than 500 patients a day in the United States. This program is peer-reviewed, fully transparent and free to the public. Grades are updated twice annually, in the fall and spring.
