If you’re already participating in our hospital referral panel and need to enroll, fill out the form below.
For all providers within your specialty participating in the referral panel, the hospital has added a requirement to activate the CareConnect platform within your office to receive appointment requests digitally. Once enrolled, your office will automatically receive the referring provider’s name and location, the referral reason, and patient contact details when a patient is discharged from one of our ERs or UCCs with a referral from their treating physician.
To remain active on our hospital referral panel, enrollment must be completed within 30 days and you must complete the attestation below. Your physician relations representative will be in touch during that window to support your activation.
Platform requirements
In partnership with the Capital division, I certify that our practice will make every effort to schedule patients with the referred to provider within 7-10 days. In the event scheduling with the named provider is unable to be accommodated, our practice will attempt to contact the patient to assist them with seeing a different provider if that is the patient’s choice.
